Eight Tips on How to Boost Your Fertility

October is National Infertility Awareness Month. It goes without saying that we appreciate the opportunity we have to educate people about various issues and current trends. The 2009 annual meeting of the American Society for Reproductive Medicine will be held in Atlanta October 18 – 21st. I want to share some tips on how to increase your fertility. This issue affects approximately 7.3 million women and men in the United States, representing 15% of families in their reproductive years.

AGE

I want to start with age, because it is one of the biggest. A woman is born with 1-2 million eggs and by the age of 37 she has 25,000. How well you reproduce depends on several factors, but one of the most important is age. The quality of the eggs begins to deteriorate after 30, and especially after 35, due to the decrease in egg production. There is no way to change the egg yolk. Consider trying to start your family sooner rather than later. If that’s not possible, staying on birth control pills can help the eggs work in the woman’s intestines by keeping the eggs in place. Seek help for early infertility issues rather than making them a priority. If you are over the age of 35, create a well-known health plan that has a regular schedule. Don’t let your doctor reassure you that “everything is fine and you should just wait it out.” Basically I advise my patients over 35 years old, if there is no pregnancy after 6 months, see a fertility specialist for a full evaluation (which only takes 1-2 months to complete), make a treatment plan in the prescribed period and move on. through that system in sequence. For example, if you start with induction of ovulation and sex, use it for 4-6 months, then go to IUI for only four cycles, then go to IVF. As time goes on, your chances decrease, even with IVF.

LIFE

The most important thing here is good health and exercise years before trying to conceive. A minimum of five hours of physical activity each week is recommended. Good habits start early, but it’s never too late to start! Being overweight can also affect your ability to get pregnant, in some cases, and being above or below your ideal BMI (body mass index) can be detrimental. FOOD

Good nutrition before you start trying to conceive is also important. It makes sense to eat more fruits and vegetables, but did you know that milk and yogurt have been found to be important? A study by Harvard Medical School shows that whole milk, not skim, is the best protection against ovulatory infertility. Another interesting finding has been that folic acid supplementation improves ovulation in women, and in men, sperm quality! It can be taken as a multivitamin and is found in foods such as oranges. Removing body fat in women with diabetes appears to help. Consuming too much caffeine and alcohol is also important. Again, the right weight is beneficial. No matter what anyone says, there are no diets or foods that “help” fertility, but a healthy diet can definitely help.

INTERMEDIATE SEX

Many couples trying to conceive use ovulation kits. There are some things you should keep in mind. Most women ovulate 14 days before menstruation. For example: If your cycle is 25 days, then you ovulate around day #11. Your fertile period will be CD #9-13. Those are the days I would encourage sex. You should stop having sex on CD #7 and wait until CD #9 starts. Have sex once a day for the five days, ejaculate once a day. Start using your ovulation kit on CD #9 (counting 16 days from the end of your cycle). Remember that when the egg is released from the ovary, it only receives the sperm and can meet the sperm for about 12 hours. If you have irregular cycles you may have another problem and you should see a specialist to find out what is going on. But the bottom line is this, make it fun NOT scientific!! You men will be reluctant participants if forced. Don’t tell him, “honey its my fertile time again we have to have sex”, on the contrary, he shouldn’t know. You just need to set the stage to make him interested, happy and “proud.” That way, you’ll both enjoy the experience, and testing won’t be a chore.

STOP smoking

Almost all studies show that smoking impairs fertility. For women, 10 or more cigarettes a day can reduce ovulation. Smoking after pregnancy is associated with miscarriage and ectopic pregnancy. Men who smoke have problems with lower sperm count and lower sperm motility and, in other words, lower sperm function. The worst is smoking marijuana. Any drug that enters your body, goes into your blood, into your cells and into your sperm and/or eggs. This is an absolute no no! The same is true of other types of recreational drugs including alcohol. My rule of thumb is, if it affects your brain cells, then it affects your reproductive cells.

KEEP YOUR HEART DOWN

Couples who want to get pregnant can feel anxious, especially if they have been trying for several years. Yoga tapes, acupuncture, massage and meditation tapes designed for infertility patients all help. My patients are encouraged to use relaxation techniques. It helps them through the ups and downs of the IVF process. Patients approach the day of the procedure in a calm, relaxed manner and it can make a difference in how recovery and delivery go. Going to see a massage therapist or meditation may also be covered by insurance, if it can be shown that there is an anxiety disorder. As mentioned above, make it fun and interesting, not homework.

BE YOUR OWN DIRECTION

You need a doctor who knows how to continue childbirth. Most of these types of doctors will be able to provide ALL aspects of infertility treatment. Just like you don’t want a doctor who only does Clomid, you don’t want a doctor who only does IVF. You will be pushed into the only thing they can do for you, Clomid or IVF. It is easy to visualize this. . . just ask, “what levels and types of support can you give me?” Most importantly, infertility should be diagnosed and treated by a Doctor, not physicians, nurses, PAs or medical assistants.

IF ALL ELSE FAILS…

If your fertility journey is facing many obstacles, then you may need to do some research. Remember, that if you are unable to achieve pregnancy naturally, it is not the end of the road. I advise my patients, “These days, we can get pregnant with almost anyone, it just depends on what I have to do to achieve the pregnancy”. There are many reasons why women or men are infertile and a visit to a fertility specialist can reduce or confirm the source of the problem. Treatment often varies from person to person so don’t expect to find an answer in a forum or forum! Blocked fallopian tubes, high FSH, abnormal or low sperm count are all common causes of infertility in many couples.

A diagnosis by a doctor is essential to determine the best course of action for couples trying to conceive. But, don’t let your doctor put you on Clomid without a good reason. Clomid is NOT a miracle drug. It has a specific purpose, which is to induce ovulation in women who do not ovulate. If you have regular cycles, that is a sign that you are ovulating. This means that the problem is something else and Clomid will not change it. Make sure that the treatment you are receiving addresses a specific problem. Ask the doctor to explain his method, why he is using the medicine he recommends and what he is treating. Treatment without a valid reason is a waste of time and money, as is treatment without completing a full evaluation of infertility. I can’t tell you how many patients I’ve seen who have had multiple rounds of Clomid or IUI only to find out that their tubes are blocked!
