Women and Heart Disease – What Most Doctors Don’t Know

If you are a woman experiencing chest pain or shortness of breath, it will take longer to be evaluated by EMS and transported to the emergency room than if you are a man. This was reported in this month’s Circulation: Cardiovascular Quality and Outcomes, and was summarized for the general public in the New York Times. What’s even worse is that even when women get to the emergency room sooner, heart attacks are made at a slower rate than men.

What this article doesn’t say, however, is how to prevent heart disease in women in the first place. Sleep apnea is a very common condition that if left untreated, can lead to anything from depression, anxiety, high blood pressure, diabetes, obesity, heart disease, heart attack or stroke. It is estimated that about 1/4 of all men and 1/10 of all women may have some form of sleep disorder. An alarming statistic is that nearly 90 percent of women with sleep apnea are undiagnosed. Instead they are being treated for the complications of sleep apnea, not the cause itself.

There are three main factors related to women that prevent the correct diagnosis of sleep apnea: the myth of insomnia, hormonal imbalance, and sleep problems.

Myth #1: Sleep apnea is mostly a male problem

One of the biggest myths about sleep apnea is that you have to be middle-aged and older, overweight with a big neck. This view is common among many doctors, so unless you are at the very end of the disease, sleep apnea cannot be considered a possibility if you are a woman. When obstructive sleep apnea was first described in the 60s, it was first described in older, more tired men. Even today, medical teachers continue to post pictures of these patients, and sometimes, a picture of Joe the Fat Boy from Dickens’ The Pickwick Papers. Paper after paper continues to describe the relationship between weight, neck size, and male sexuality and the increased risk of sleep deprivation.

However, it has recently been shown that even small, thin women who are not very active can suffer from sleep apnea. I see patients like this every day in my practice. This also applies to skinny guys. Whenever these people get a formal sleep study, they always have their doctors reply, “you don’t look like you have a sleep disorder.” This just goes to show that there is no “known” sleep apnea sufferer.

Myth #2: Men Can’t Stop Showering

It is often known that when women go through menopause, their heart disease starts to meet with men (although not completely). Common symptoms of menopause include night sweats, hot flashes, mood swings, weight gain, insomnia and irritability. However, as I mentioned before, guys in their 20s can have similar symptoms. How is this possible? It is because these men, while losing weight little by little, continue to breathe continuously, and little by little they start to suffocate. It is a gradual change in the body that disrupts the autonomic nervous system, causing so-called vasomotor symptoms. Your autonomic nervous system is what controls your heart rate, blood pressure, body temperature, sweating, digestion, and sleep.

As women approach menopause, one of the first changes they go through is that progesterone levels gradually decline starting in their mid-40s. One of the most interesting properties of progesterone is that it acts as a natural stimulant and stimulant. This is what causes pregnant women to breathe hard and fast. It also strengthens your upper body muscles by stimulating more muscles.

As I point out in my book, Sleep, Disruption, most human language today takes a bit of a backseat when we’re on our backs. When you increase muscle relaxation during deep sleep, our tongues can collapse and obstruct our breathing. This is why many people cannot sleep on their backs. So if you take a little progesterone in women, the tongue relaxes a lot, and wakes up a lot, usually from a deep sleep. Not getting enough sleep promotes weight gain, and weight gain makes the throat worse. Many of these obstacles and arousals lead to disruption of the nervous system. After that, a few breaths after one of the blockages gets worse, the breathing starts to decrease, and the blockage of sleep gets worse.

This is why many women sleep better when taking hormone replacement therapy, and have a lower risk of heart disease. Now, because of the conflicting studies regarding hormonal changes and the incidence of cancer, this is a difficult and controversial topic that cannot be explained. However, post-menopausal women have been using progesterone cream for years to improve their quality of life and sleep.

Myth #3: Lack of sleep affects everyone the same

Finally, a previous study showed that women who complain about sleep problems to their doctors are diagnosed with insomnia more often than men. In many cases, these women were given sleeping pills. Men who complain about sleep problems are often asked if they burn, and it is fair to say that insomnia is the most common problem in men. Again, this brings up the idea of OSA that doctors have in men versus women.

There are a number of other reasons why women tend to be less well cared for than men. I have listed only three reasons that prevent women from early diagnosis of sleep apnea. Not receiving treatment to prevent insomnia can lead to heart attacks. As long as the medical community divides diseases without looking at the whole picture, we will continue to have inconsistencies in the care of not only women, but also men.
